Goal: The purpose of California Main Street is to educate and provide assistance to California communities about how they can revitalize their downtowns or core commercial districts using the self-help Main Street Four-Point Approach. Through this education on how traditional downtowns and neighborhood commercial districts can become and remain vital economic, social, and cultural centers, the community's development leads to livable and sustainable practices that ultimately improve its quality of life.

Goal: The goal of this workshop is to build the participants’ self-confidence in their ability to manage their health and maintain active and fulfilling lives despite their chronic disease condition.

Impact: In 2016 Derby Health Collaborative hosted six free self-management education courses in partnership with the Derby School District, the Derby Senior Center, the Derby Public Library and the Kansas State Extension and Research Center. Workshops will continue in 2017.

Goal: The Community Market Farms program transforms vacant land into market farms and public spaces in order to grow and distribute organic affordable food with and for the community.

Goal: To assess the costs of a housing and case management program in a novel sample: homeless adults with chronic medical illnesses.

Impact: Compared to usual care, the intervention group generated an average annual cost savings of (−)$6,307 per person (95 percent CI: −16,616, 4,002; p = .23). Subgroup analyses of chronically homeless and those with HIV showed higher per person, annual cost savings of (−)$9,809 and (−)$6,622.

Goal: The goal of Sojourner Health Clinic is to pick up where the current medical system leaves off by providing free acute and ongoing healthcare to patients who do not have access--or are reluctant to access—traditional routes to a doctor or medicine.

Impact: During 2012, the Sojourner Health Clinic managed to increase the number of diabetic patients with A1C levels below 9 percent from 15% to 83%, reducing the average blood glucose level of patients and giving them a better sense of control over their diabetes.
